1.Pick up a blue form from the office. Fill out and hand in here at PVEA.
2.Get packet from office and make appointment at the district office for Livescan fingerprints.
•Tuesday 7:30 am - 10:30 am
•Thursday 12:30 pm - 3:30 pm
3.The Fingerprint service costs $50. A limited number of vouchers are available for those who need financial assistance.
4.A TB test is required every 4 years.
